Vendor Registration for Shalomar Production Conferences and Events

            I/we wish to register to be a vendor at Prophecy, New Science and the Days to Come, put on by Shalomar Productions.

            I/we will be solely responsible for everything that I/we bring to sell. I/we will be responsible for setting up, taking down, providing all tables, gazebos and anything else that I might require, including security. I/we will not hold Shalomar Productions responsible for any losses or damages that I/we may incur. If not made available on-site, vendor agrees to provide own safe and adequate power supply needs.

            In exchange for my participation at this/these conferences I will remit to Shalomar Productions by check or money order the sum of $200 for each event.
